Certificate of Insurance
A document issued by an insurance company/broker that is used to verify the existence of insurance coverage under specific conditions granted to listed individuals. Specifically, the document lists the effective date of the policy, the type of insurance coverage purchased, and the types and dollar amount of applicable liability. To request a change to your current insurance policy, please call one of our friendly sales agents. No coverage or premium adjustment of any kind is bound until you receive written notice.
Auto Policy ID Card Request Form
If your Auto Policy ID is lost or stolen, call Mary Ann at 513-984-0404 extension 102 to request a replacement card for your current insurance policy. The Auto-ID Card provides proof of coverage and also provides the insured with information on what to do in case of an accident. Certain states require an Auto ID Card be carried in the vehicle at all times.
